On a multi page site, yes, because it pays for your hosting, SSL, business email and domain renewal every year. It is not a contract though. No minimum term, cancel any month, and you keep your domain and your files. On a single landing page the monthly is optional.
Yes. Most clients are in San Jose and the South Bay because that is where I am, but the work does not require me to be local. If you are somewhere else and the fit is right, that is fine.
